Pharmacy Services in a Changing 340B Landscape

The Episcopal Health Foundation retained Health Equity Outcomes (HEO) to conduct the analysis summarized in this report to assist its plans for supporting FQs in its service area, to protect and (where feasible) expand pharmacy services, access, and revenues. This analysis included three phases: (1) a literature scan to assess the national perspective on pharmacy structures, services, and revenues among FQHCs and Look-Alikes and relevant trends in 340B (2) an e-survey of leaders of FQs and Look-Alikes in EHF’s district and (3) semi-structured qualitative interviews of FQ and Look-Alike Leaders in EHF’s District and TACHC.

As leaders of FQs anticipate the continued evolution of the 340B Drug Discount Program and the resulting impact on the financial viability for their organizations, it will be critical to rapidly identify and execute strategic investments to protect this crucial component of the safety net across the state of Texas. When integrated into health center operations, pharmacy services can provide an important revenue stream for FQs, and support greater patient engagement and continuity of care with providers. However, the provision and expansion of these services have been limited by existing staff capacity and internal expertise. This report highlights specific opportunities for collaboration among innovative FQs, supporting stakeholders, and subject matter experts to drive high-quality, affordable, accessible care for patients and communities across the state of Texas.
